Om kroppen tillförs stora mängder syntetiskt könshormon, finns risk för allvarliga biverkningar. Hjärtat kan påverkas på flera sätt. Vänsterkammarhypertrofi, kardiomyopati, arytmier, accelerad kranskärlssjukdom och hjärtinfarkt har rapporterats, i vissa fall hos unga män under 30 år.
Ökad risk för trombos (blodpropp) förekommer. En ökad blödningstendens har noterats i samband med operativa ingrepp. En ökad bildning av röda blodkroppar, erytrocyter och samtidig användning av diuretika (vätskedrivande medel) kan ge polycytemi (för många röda blodkroppar i blodet). Även trombocytfunktionen kan troligen påverkas. Näsblödningar och hematospermi (blod i sädesvätskan) förekommer. En ökning av det "dåliga" kolesterolet LDL samt en sänkning av det "goda" kolesterolet HDL är konstaterat. Leverenzymvärdena är ofta förhöjda och vätskeansamling (ödem) är vanligt förekommande. Hur kroppen påverkas av AAS beror på individuella arvsanlag, metabolism, dos och under hur lång tid preparaten använts.
